[Rd] Connections patch

I've implemented an R Connections API for packages writers and those embedding R into other systems. The patch is here:

and I've started an R wiki for discussion here:

Main points:

  1. C code can create new connections and pass them back to R.
  2. R code can utilize them just like existing connections.
  3. C code can also utilize them.
  4. stdin, stdout, and stderr connections can be replaced with something else.
